My Adoption Plan & Process

The adoption process in Ohio is very simple.

The first step is to contact Choice Network. Together we will review your options and make sure adoption is the right choice for you. We will also review the adoption process in Ohio and begin to develop your adoption plan.

If you decide that you would like to go forward with your adoption plan, we will help you complete the Social/Medical History form (1616) so that the adoptive parents will have a medical history of the child.

You will also complete a form which permits or denies your child the right to obtain identifying information about you when the child turns 18.

In your adoption plan, you will decide what level of openness you prefer. This may range from a closed adoption; to a semi-open adoption; to an open adoption (and anywhere in between). Choice Network supports birth families choosing open adoptions.

Next, you will be given profiles of adoptive families that match your description of the perfect family for your child. You may select a family from reviewing these profiles or through interviews.

After the birth occurs, Ohio requires a 72 hour waiting period so that you can be certain that you want to go forward with your adoption plan. Your child will remain in the hospital during this time. After 72 hours, we will visit you either at the hospital or in your home so that you can sign the Permanent Surrender document. Once this is complete, your child goes home with the adoptive parents directly from the hospital.

We will remain available to you to provide counseling for as long as you request at no charge to you. We are here to help you complete the adoption plan that you make for your child and to help you to continue to feel proud that you chose the decision that was right for you.
